Practical instruments for everyday educational audiology: questionnaires, screeners, and interactive tools you can put to work with students and patients. The free resources are open to everyone and interactive versions (when available) are a benefit of EAA membership.

Screening Questionnaire

Buffalo Model Questionnaire–Revised (BMQ-R)

A 48-item screening questionnaire for characteristics associated with auditory processing disorder (APD), organized by the Buffalo Model categories. The questionnaire and manual are free to download; members can also use the interactive version with automatic scoring and a formatted PDF for the patient record.

Using These Tools

These tools are intended for use by qualified professionals as part of a complete evaluation, together with appropriate test batteries, case history, and clinical judgment. Interactive tools run entirely in your browser: no patient information is collected, transmitted, or stored by EAA, and professionals remain responsible for handling protected health information (PHI) in accordance with FERPA, HIPAA, and their organization’s policies.
